Let us bring healing to you with a Lifestyle Medicine home visit tailored to your individual health needs. This service will not be available to attend to urgent or critical care needs. A Board Certified Lifestyle Medicine Physician and Registered Nurse will come to your home and offer any variation of the following interventions and services within the 1 hour appropriated time: 1. Physical Exam 2. Biometrics 3. Vital Signs 4. Labs with immediate results: Lipid Panel, Hemoglobin A1C, Bood Glucose (cost is included in the booking fee) 5. Disease Specific Education with evidence-based lifestyle interventions. 6. Nutritional Assessment (with education and recipes) 7. Pantry Makeover 8. Guided Mind-Body Practice 9. Healing Touch with Essential Oils 10. Physical Activity
