Service Description
A medical visit with a Lifestyle Medicine approach. Most insurances accepted for this service. Cash pay price available as well. This 1 hour, one-on-one consultation with a Board Certified Lifestyle Medicine Specialist includes: 1. In-depth review of Past Medical History 2. Complete Physical Exam 3. Biometrics 4. Vitals signs 5. Nutritional Assessment 6. Pertinent Labs (with capability to draw lipid panel, blood glucose, and Hemoglobin A1c in clinic with immediate results) 7. Intense Patient Education with Evidence-Based, Lifestyle Recommendations New incoming patients are asked to complete our Lifestyle Medicine Intake Assessment form (found under "New Patient Forms" prior to the visit. You can choose either the digital or printable format. We will receive the digital submission immediately. Please have this submitted to the office 1 week prior to your appointment time so that the Lifestyle Medicine team is able to review it and better prepare for your visit. We would also appreciate old medical records and labs to be brought to your initial visit.
